2. How to Apply for a membership?
Let us know on your next visit, to discuss your membership and the feasible method for payment. You can also submit your inquiry online to us via our form here.
3. How long is my membership valid?
The membership is strictly valid up to the date present on your membership card. Generally, for a one-year membership, it's 365 days from your issue date.
4. How to Make a Booking?
Booking should be made at least 1 day before your appointment date with your preferred time & therapist or doctor online via ChaitanyaHealth.com
5. How to redeem your Membership?
When you become a member at Chaitanya Health & Wellness, you get a membership card as defined by the type of your membership package. You will also have a unique member code eg. VASZCGSJ which will be used to keep track of your membership. Please be present at the front desk with your membership card and your required documents for identification.
6. How to redeem a Membership Card for your near & dear?
To redeem the membership card for your near & dear - friends, family & guest, they must be physically present and must use the service to enjoy the discount.
7. What to do if your membership card is lost/found?
If you have lost or found the membership card, kindly contact us. We will take the necessary steps to get you your membership card.
